• 1. Access the Jotform

  • Click on the designated Jotform link provided by the office or scan the QR code at the job site.
  • The form will open in your browser, ready for completion.

• 2. Enter Job Details

  • Fill in the Job Name, Location, and Date.
  • Select your team members from the drop-down list or enter their names manually.

 

• 3. Identify Hazards & Control Measures

  • Review the pre-listed work activities and associated risks.
  • Tick the applicable control measures for each hazard.
  • If needed, add any additional hazards or safety controls.

 

• 4. Acknowledge PPE & Safety Requirements

  • Confirm that all required Personal Protective Equipment (PPE) is available and being used.
  • Acknowledge any site-specific safety requirements
    

 

• 5. Sign & Submit

  • Scroll to the bottom and use your touchscreen or mouse to sign the form digitally.
  • Click Submit, and a copy will be sent to the office for record-keeping.
